22VAC40-191 Background Checks (22VAC40-191)

The licensing inspector conducted an unannounced monitoring inspection on 08/23/2018. The children were observed playing on the playground, reading, playing with toys, and participating in small group activities. A formal observation was completed with the preschool and school age classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playground was inspected today as well as the center's mini bus. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Six injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/30/2018 Last shelter-in-place drill: 11/16/2018 Last fire inspection: 04/09/2018 Last health inspection: 08/23/2018 Last lockdown drill: 03/14/2018 Today, the following child to staff ratios were observed: Toddlers / Twos ? 14:3 Preschool ? 8:1 School Age - 11:1 The violations from the previous inspections were checked for corrections. Description: Based on record review, the center did not ensure that one of five children's records had documentation of a physical examination by or under the direction of a physician. Evidence: The record of child #3 (start date: 01/22/2018) did not have documentation of a physical examination.

Plan of Correction: Will retrieve physical from mom by Monday, August 27, 2018.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not ensure that one of five children's records had all of the required information. Evidence: The record of child #3 (start date: 01/22/2018) did not have the following required documentation: address and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Will retrieve emergency contact from parents Thursday, August 23, 2018.

Standard #: 22VAC40-185-340-D
Description: Based on observation and an interview, the center did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director was regularly present.
The record of staff #5 (start date: 09/13/2016) did not have documentation confirming program leader or director qualifications. The center representative confirmed that staff #5 did not have documentation of program leader or director qualifications. The licensing inspector observed staff #5 alone in a classroom with 11 school age children during the inspection.

Plan of Correction: Will make sure program lead is in class at all times.
