The licensing inspector conducted an unannounced monitoring inspection on Thursday, August 16, 2018 from 10:05am to 2:50pm to determine the center's compliance with licensing standards. During this inspection, the center's director was present and assisted the licensing inspector throughout this inspection. The census for today's inspection consisted of 112 children in care and 18 staff. During the inspection, the licensing inspector observed the children playing with toys, reading books, and doing crafts. The children were also observed playing outside and during water play. The observed staff/child interactions were positive and supportive of children's growth and development. The menu for lunch consisted of chicken tacos, Mexican corn, orange slices, and milk. The following information was reviewed: program activities, supervision, staffing, interactions with children, daily routines, menu, posted information, emergency supplies and drill records, and agency inspection reports. The last documented health inspection occurred on June 21, 2018. The last documented fire inspection occurred on June 4, 2018. There were eleven children's records and ten staff's records reviewed during this inspection. The center's first aid kid and emergency supplies were inspected and found to be in compliance. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from today. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-60-A Description: Based on a review of eleven children's records on 08/16/2018, the center did not ensure that five out of eleven records reviewed had annual parent confirmation that the required information in the child's record is up-to-date. EVIDENCE: 1) The record of Child #1 (DOA: 9/14/2015), Child #4 (DOA: 04/24/2017), Child #6 (DOA: 02/02/2016), Child #7 (DOA: 07/07/2014), and Child #8 (DOA: 01/04/2017) did not contain documentation of child updates and confirmation of up-to-date information in the record as required by 22 VAC 40-185-420 E 3. Plan of Correction: Per the Director: Parents are emailed and paper put into cubby to update information. Yearly plan put into place and tracked for returned documentation.
Standard #: 22VAC40-185-270-A Description: Based on observation of the center on 08/16/2018, the center did not ensure that areas of the center shall be maintained in operable condition. EVIDENCE: One of the two toilets in the Pre-K 1 classroom does not work properly. The inspector and the director attempted to flush the toilet during the inspection, but it did not flush. The inspector observed a child using this toilet later in the day on the day of this inspection. Plan of Correction: Per the Director: Ticket put in at 11:48 am for plumber to repair.
Standard #: 22VAC40-185-280-B Description: Based on observation of the center on 08/16/2018, the center did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: A can of air freshener was observed on a counter top in the Pre-School 2 classroom which was accessible to the children in care. Plan of Correction: Removed from the classroom. Corrected on site.
