22VAC40-185 STAFFING AND SUPERVISION.
Today a complaint investigation was conducted regarding concerns of a child being left alone in the restroom for 8-10mintues. After carefully, interviewing all involved staff this complaint was deemed valid. If you have any questions, please e-mail me at Mayasah.Thomas@dss.virginia.gov
Standard #: 22VAC40-185-340-F Complaint related: Yes Description: Based on interview with staff, center failed to provide sight & sound supervision for a child under the age of 10 who was sent to use the rest room. Center staff failed to check and provide assistance to child after five minutes when the child did not return. Evidence: Staff #1 was in the office area cleaning the lunch bowls when Child #1 was sent over to use the restroom. Staff #1 stated that the child was in the bathroom for a while and returned to the classroom to assist with cleanup. Staff stated that she went back and forth to check on the child, however she noticed the door of the bathroom was closed Staff #1 assumed that th child had returned back to the classroom and was finished using the restroom. Staff #2 confirmed that approximately 10 minutes later the child returned to the classroom, and when they asked where she was "Child #1 stated, i was in the bathroom and you left me alone". Plan of Correction: DIrector reviewed with staff the supervision guidelines. Reminded staff that children "always shall be within actual sight and sound supervision of staff". Discussed where staff shall stand while waiting for a child who is using the bathroom.
