We discussed playground equipment and resilient material. Discussed diapering surfacing.
An unannounced monitoring visit was conducted August 17 2018 between 11:30am and 3:15pm. There were 90 children observed in the direct care of 15 staff members. Children were observed eating lunch, playing on the playground, washing hands, and napping. Center was observed to be clean, organized, and offer plenty of age appropriate materials for the children to utilize. Areas of non-compliance are listed on the violation notice. Standard #: 22VAC40-185-210-A Description: Based on review of staff files, 1 out of 5 did not have documentation indicating they completed a high school program. Evidence: Staff 1 has not received a high school diploma or equivalent. Graduation is planned for December 2018. Plan of Correction: We will take her out of the room for the next two weeks.
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not in a locked place. Evidence: Room 9, bleach and water solution was observed on top of a cabinet. Plan of Correction: Corrected at time of visit by placing in a locked cabinet.
Standard #: 22VAC40-185-280-G Description: Based on observation and interview with Director, substitute container containing a hazardous substance was not labeled. Evidence: Room 6, spray bottle containing cleaning solution was not labeled with contents. Plan of Correction: Corrected at time of visit by removing the bottle from the room, and all bottles will be labeled or in original containers
Standard #: 63.2-1720.1-B-3 Description: Based on staff file review and interview with Director, 1 out 5 files did not have documentation of out of state Central Registry requests for any state other than Virginia the staff has resided in the preceding 5 years. Evidence: Staff 2 was missing Central Registry request for Texas and Pennsylvania. Plan of Correction: They will be completed and mailed out today.
