Comments:
An unannounced monitoring inspection was made on 8/21/18. There were 23 children present with three staff supervising. Five children's records were reviewed. The inspection was completed at the main YMCA on 8/22/18. Four staff records were reviewed on that date. The inspector arrived at the school at 3:00 pm and departed at 4:00 pm on 8/21/18. The inspection to review records on 8/22/18 began at 10:00 am and ended at 12:30 pm. The children were observed during snack and during homework time.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to ensure that each staff person had an updated tuberculosis (TB) screening or test at least two years from the date of the last screening or test. Evidence: 1. The last documentation of a TB test for staff #1 was 7/22/16.

Plan of Correction: The staff person sent a picture of the completed TB test to the child care director during the inspection on 8/22/18.
