Technical assistance was provided regarding medication authorization forms, storage of medications and appropriate authorization of medication. A discussion was held in reference the possibility of keeping the front door locked.

An unannounced monitoring inspection was conducted today from 9:15am-2:00pm. There were 65 children supervised by 10 direct care staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, and policies were inspected. Children were observed participating in group play, circle time, reading books, playing outside, and singing. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-130-A Description: Based of review of children's records, the center did not obtain an immunization record before a child started care. Evidence: Child #4, start date 8/20/18, does not have an immunization record available during the inspection. Plan of Correction: I will get a copy of their immunization record.
Standard #: 22VAC40-185-160-A Description: Based on review of staff records, the center did not obtain a tuberculosis (TB) screening, no later than 21 days of hire. Evidence: Staff #1, date of hire 5/1/18) does not have record of a TB screening on record. Plan of Correction: I will make sure she gets her TB screening completed.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the center does not have a complete record for all employees. Evidence: Staff #1, date of hire: 5/1/18, is missing the follow from their record: emergency contacts, H.S. diploma , orientation, and their qualifications for the position. Plan of Correction: I will get all the necessary information from the staff member.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas of the center are maintained in a clean, safe and operable condition. Evidence: In the school-age classroom, peeling and chipped paint was observed in a location that is accessible to children. On the large playground, broken and cracked toys, some with sharp edges, were accessible to children. Plan of Correction: We are in the process of being re-painted. I will clean up the playground and remove all broken toys.
Standard #: 22VAC40-185-340-J Description: Based on observation, the center allowed a child to leave the center unsupervised. Evidence: Child # 5 left the classroom unsupervised, walked down the hall, out the front door, ran through the parking lot and sat on the sidewalk. Office staff immediately ran after him, meeting him at the sidewalk. Plan of Correction: Director will discuss options with family.
