Comments:
The local agency representative and licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 07/23/2018 regarding allegations of procedures for emergencies. The inspection was initiated on 08/01/2018 and completed on 08/28/2018. On August 1, 2018, the inspector was on site from approximately 9:56 am to 10:55 am. Interviews with staff were completed, observation of physical plant, staff and child records, and documentation were reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there was a violation found during the inspection. See the violation notice for more details. Description: Based on review of staff record and staff interview, the center did not ensure each staff member submit documentation of a negative tuberculosis screening within 12 months prior to or 21 days after employing or volunteering. Evidence: During the inspection conducted on 08/01/18, a review of Staff#1's record (date of hire 06/17/16) was completed and there was no documentation of TB screening found. Interview with staff confirmed that documentation of TB screening had not been submitted by the staff at this time.

Plan of Correction: Staff will have TB screening completed by 08/08/18.
