An unannounced non-mandated monitoring inspection was conducted on August 13, 2018 in response to a self reported incident reported to the Valley Licensing Office on August 7, 2018. The report concerned an injury to a child. Interviews were conducted, observations made and relevant/available documents were reviewed. No violations of the licensing standards were cited as a result of this inspection.
