Comments:
An unannounced monitoring inspection was conducted from 8:30am until 11:45pm. At the time of the tour there were 91children in care with 17 staff present. A sample of 2 staff and 9 children's records were reviewed. There were no children with food allergies at the time of inspection. Children were observed outside on the playground, being read a story and playing in centers. Infants were observed playing on the floor with staff and being fed.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method. Evidence: In the 3 year-old room there was a wooden box attached to the wall containing a bottle 1 bottle of disinfectant spray and 1 bottle of multisurface glass cleaner. The box was not locked. The director will monitor for compliance.

Plan of Correction: The following corrective action was provided by the program director: The staff will ensure the boxes are locked at all times.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not ensure all diapering requirements were met. Evidence: A staff in the infant room was observed changing a baby's diaper. The staff member did not discard of the soiled diaper and left it on the changing table. The child was changed and placed on the floor. The staff member then proceeded to throw away the soiled diaper in a trash can with a lid. The foot operated trash can was not used and the staff's hands touched the the lid as she discarded the soiled diapers.

Plan of Correction: The following corrective action was provided by the program director: Director will remind staff of the proper diaper changing procedures. The director will monitor daily for compliance.

Standard #: 22VAC40-185-560-G
Description: Based on observation the center did not ensure all requirements were met when food is brought from home. Evidence: In the infant roo there were 2 cups that contained breast milk that were labeled 08/27/18. One of the cups had condensation on the lid.

Plan of Correction: The following corrective action was provided by the program director: The staff fixed labels immediately during the inspection. The director will check the refrigerator nightly to ensure all bottles have gone home.
