Today an unannounced renewal inspection was conducted between 7:45am & 9:30am . There were 32 children present with 5 staff. Children were observed arriving, playing cards, reading and playing games. Physical plant, programs, children's records, medications, and emergency drills were also reviewed. Staff records are reviewed annually in the administrative office, however, MAT (medication administration training) and first aid/CPR certificates were available for review. Description: Based on review of children's records, the center did not obtain documentation of immunizations for a child prior to the child's first attendance. Evidence: Child #2 (start date 8/28/18), did not have documentation of immunizations available during the inspection.

Description: Based on review of medicines on site, not all procedures were followed for medications. Evidence: Child #1 has an medication authorization form written for a name brand over the counter medication, the actual medication on site is a generic brand.
