Comments:
An unannounced renewal inspection was conducted on August 29, 2018, between the hours of approximately 10:10 a.m. and 2:40 p.m. Fourteen (14) children were in the care of three/four staff members, in two groups, during the on site inspection. Five children's records were reviewed and five staff/administrative records were reviewed and/or updated. No medications were on site/administered on this date. Posted information (license, licensing inspection documentation, emergency evacuation diagram and plan, emergency telephone numbers, snack menu and classroom schedule), safety equipment (first aid kit, flashlight and radio) and available documents (emergency evacuation drill and shelter-in-place drill records, one local agency report, attendance record and injury records) were reviewed. The daily schedule accurately reflected the activities provided. Lunch was provided by the children's parents. Program observation was completed. Activities observed on this date included: outdoor play; indoor play; group time (songs and learning concepts - months of year, days of week, numbers and alphabet); group game (Bingo); diapering, restroom and handwashing procedures; lunch; stories; naptime and arrival/departure of children. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Children's artwork was displayed. Follow-up was completed on August 31, 2018 between 8:00 a.m. and 9:00 a.m. Previously cited violations were reviewed for compliance (2/15/2018 inspection date); two repeat violations were cited. Exceptions to compliance with the licensing standards were noted on the Violation Notice. Recordkeeping was discussed (maintaining complete and up-to-date records). Standard #: 22VAC40-185-160-A
Description: Based on review/update of five staff and administrative records and discussion with staff on August 29, 2018, the program failed to obtain documentation of negative tuberculosis (TB) screenings for two staff members that were completed within 12 months prior or 21 days after employment. Evidence: 1. Review of staff #3's record, on this date, determined that the TB screening documentation submitted was completed more than 12 months prior to employment (employment date - 3/01/2018). 2. Review of staff #4's record, on this date, determined that a TB screening was not on file (employment date - 5/14/2018). 3. Staff #1 confirmed the date of staff #3's TB screening and the dates of employment for staff #3 and staff #4.

Plan of Correction: TB screenings will be obtained for the staff members. In the future, documentation of negative TB screenings will be obtained within 12 months prior or 21 days after employment.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of one applicant/agent background clearances and discussion with staff on August 31, 2018, the program failed to obtain an updated sworn statement and central registry clearance at least three years from the date of the last clearance. Evidence: 1. Sworn statement has not been updated and central registry clearance for applicant/agent #5 was dated more than 3 years after the date of the last clearance (sworn statement - 10/28/2013 and central registry clearance - previous 11/22/2013 and current 3/29/2018). 2. Staff #1 confirmed the dates of the sworn statement and central registry clearance.

Plan of Correction: A new sworn statement will be obtained. In the future, background clearances will be updated in a timely manner.

Standard #: 22VAC40-191-60-C-2
Description: Based on review/update of five staff and administrative records and discussion with staff on August 29, 2018, the program failed to obtain central registry findings for one staff member within 30 days of employment. Evidence: 1. Review of staff #4's record, on this date, determined that central registry findings were not on file (employment date - 5/14/2018). There was no documentation on file indicating the central registry request had been submitted within 7 days of employment.

Plan of Correction: The central registry request has been submitted and follow-up was completed after 30 days. The central registry results will be obtained. In the future, documentation will be maintained indicating when the central registry request has been submitted and when follow up has been completed.
