Comments:
The licensing inspector conducted an unannounced focused inspection on Friday, August 3, 2018 from 11:40am to 12:47pm. This inspection focused on the main areas of previous non-compliance. The children were observed eating lunch. The following child to staff ratio was observed: Infant-5:1 Infant II-6:2 Toddlers-7:2 Two's-19:3 Preschool-10:1 Pre-K-17:2 School Age-26:2 If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records on 08/03/2018, the center did not ensure that four of the five records reviewed contained documentation that two or more references as to character and reputation as well as competency were checked before employment. EVIDENCE: 1. The record of Staff #1 (date of hire-03/15/2018), and Staff #2(date of hire-12/08/2017) did not contain documentation of reference checks. 2.The record of Staff #5, whose date of hire was 10/08/1987 contained documentation of one reference check.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 08/03/2018, the center did not ensure that over-the-counter skin products shall not be kept beyond the expiration date of the product. EVIDENCE: An expired tube of diaper cream (Baby Butt Paste) belonging to Child #6 was observed in the Infant II room. The cream expired 07/20/2018.

Description: Based on observation on 08/03/2018, the center did not ensure that staff shall sit with children during meal times. EVIDENCE: The staff in the Infant II class were standing and putting out cots while the children were at the table eating lunch.
