22VAC40-191 Background Checks (22VAC40-191)

An unannounced renewal inspection was conducted from 9:22am-2:00pm. At the time of entrance, 230 children were in care with 52 staff members present. Children were observed painting, playing with play dough, reading books, singing during circle time, snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on review, the center did not obtain from staff at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors the results of a follow-up tuberculosis (TB) screening as stated in subsection B of this section. Evidence: 1. Staff #2 (date of hire DOH 8/26/2002) had a TB screening form dated 7/22/2016 and had been more than two years. 2. Staff #4 (DOH 8/29/2016) had a TB screening form dated 7/21/2016 and had been more than two years.

Plan of Correction: Both staff members had appointments to go to the doctor this afternoon to get the test completed.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside and outside were not maintained in a safe and operable condition. Evidence: 1. In classroom 231, a blue stool had torn fabric creating a hole. 2. On both playgrounds, the covered sandbox had at least 2 inches of standing water on the majority of top of the sandbox covers.

Plan of Correction: The sandbox covers have both been dumped. The stool will be repaired.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In classroom 160, a container of Clorox wipes was inside an unlocked top cabinet. 2. In the hall bathroom, a spray bottle of air freshener/deodorizer was on a open shelf and not in a locked place.

Plan of Correction: The bleach wipes and air freshener were moved to a locked cabinets immediately.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to cover all unused electrical outlet with protective covers. Evidence: 1. In the kitchen, one unused outlet was not covered.

Plan of Correction: The outlet was covered immediately.
