Comments:
An unannounced monitoring inspection was conducted from 11:30am -1:00pm. Upon arrival there were 77 children in care with 16 staff present. A sample of 8 staff and 7 children's records were reviewed. The center did not have any medication at the time of inspection. Children were observed during lunch, school-age children were observed during the summer camp program.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a record review the center did not ensure each staff member had a negative tuberculosis screening no later than 21 days after employment. Evidence: Staff #4 (doh 07/13/18) did not have documentation of a tuberculosis screening.

Plan of Correction: All new staff will be required to bring documentation of a current TB test to day one on boarding, effective 9/4/18. New management team has been retrained and DM will review for compliance with all new hires.

Standard #: 22VAC40-185-70-A
Description: Based on a record review the center did not ensure all required information was kept for each staff person. Evidence: Staff #7 (doh 08.29.19) did not have documentation of a job title or two references that have been checked.

Plan of Correction: New management team has been retrained as of 9/4/18 on ensuring that all references are completed before the job offer and that each file contains the job title of all new staff. DM will review for compliance with all new hires.

Standard #: 63.2-1720.1-B-2
Description: Based on a record review and observation the center did not have all required background checks. Evidence: Staff #7 (doh 08.29.19) did not have documentation of fingerprinting. Staff #7 was observed working in the center.

Plan of Correction: No staff will be allowed to begin work without fingerprint clearance effective 9/4/18. DM will request said documentation prior to all new staff starts to ensure compliance.
