Comments:
The inspector conducted an unannounced mandated renewal inspection beginning at the main office to review staff records from 10:15 p.m. until 11:00 a.m. Three staff records were reviewed. The inspection continued at the center from 11:30 a.m. until 1:15. Five children's records were reviewed. Fourteen children were in care with two staff supervising at all times. Children were observed eating lunch in the school cafeteria, washing hands, playing outside, listening to music, brushing their teeth, and preparing for rest time. Medications were reviewed. Areas of non-compliance are identified on the inspection documents. Please call your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not keep all hazardous substances in a locked place. Evidence: 1. The inspector observed lysol spray, bleach water, and other cleaning supplies on the closet floor in the children's bathroom. The door was closed but not locked.

Plan of Correction: The teacher locked the door during the inspection. Staff will check during the day to make sure it is locked.

Standard #: 22VAC40-185-330-B
Description: The center did not have the required depth of resilient surfacing on the playground. Evidence: 1. The inspector measured 2-4 inches of mulch on the playground around the slides and climbing equipment.

Plan of Correction: Mulch has been ordered.
