An unannounced monitoring inspection was conducted today, September 6, 2018 to determine the center's compliance with licensing standards. The inspector arrived at 10:30 am pm and concluded the inspection at 3:30 pm. Upon the inspector arrival children were were outside on the playground playing with balls, hula hoops and trucks. Upon returning to the classrooms, children washed their hands and had water. Later preschool children were observed playing in centers (train table, puzzles, coloring, and blocks), working on art projects (makers, collages/gluing buttons and mixing paint colors) and having music and movement (dancing to hungry sharks). Toddlers were observed in circle time reviewing colors, animals and reading books. Lunch time was also observed. Children provide their own meals (see violation notice). After lunch children had nap. Bathroom breaks and hand washing was conducted. All areas of the facility to include classrooms, bathrooms, hallways, and playground area were inspected. Classrooms have age and stage appropriate materials and supplies. The facility had the license, last inspection notice, emergency numbers, emergency evacuation procedures/maps, daily schedule and various parent information posted. The first aid kit and the non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. Staff on site have current CPR, First Aid and daily health observation training. Staff no not have current MAT training and medications are not being administered. The most recent evacuation drill was on 08/08/2018. The two required shelter-in-place drills for 2018 were completed on 07/03/2018 and 08/08/2018. Today's Ratio's: Classroom #1: (4 years) 6 children with 1 staff. Classroom #2: (3 years) 6 children with 1 staff. Classroom #3: (2 years) 3 children with 1 staff. Classroom #4: (Toddler) 4 children with 1 staff. Classroom #5: (2.5 years) 6 children with 1 staff. Fire Inspection: 04/25/2018 Health 05/17/2018 The violations from the licensing period were checked for corrections. There was one repeat violations cited during today's inspection. Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the facility did not obtain documentation of a negative tuberculosis screening at least every two years from the date of the first initial screening. Evidence: The original tuberculosis screening for staff #4 was dated 05/05/2016, exceeding the two year allowance to renew. Plan of Correction: Per the Owner: I will have the staff call today to schedule a tuberculosis screening renewal.
Standard #: 22VAC40-185-80-A Description: Based on observation and interview, the facility did not keep a written record of children in attendance. Evidence: 1. A sign in and out sheet was observed posted on the wall by the entrance of the facility with twenty-one children signed in for today, September 6, 2018 however twenty-five children were in attendance at the time of the inspection. 2. Individual classrooms did not have attendance sheets. Plan of Correction: Per the owner: I will remind parents to sign in and out and will supply each classroom with an attendance book for them to keep a daily attendance of each child.
Standard #: 22VAC40-185-340-F Description: Based on observation, the facility did not ensure children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. Staff #1 was observed leaving the toddler classroom and walking to the end of the hall, approximately twenty-six feet, to warm her lunch in the microwave. 2. The door of the toddler class was pulled close and four toddlers were asleep in the classroom unattended, out of sight and sound supervision of staff #1. 3. Staff #1 was out of the classroom for approximately one minute. Plan of Correction: Per the owner: I will post notes in each classroom as a reminder not to leave the classroom. I will review with staff that they need to have everything they need in the classroom before nap time and if they leave the classroom any other time they need to take the children with them.
Standard #: 22VAC40-185-430-K Description: Based on observation, the facility did not ensure provision were made for an individual place for each child's personal belongings. Evidence: 1. There was no individual space for children's personal belonging in the four year old classroom. Children's belongings (linens and nap time supplies) were observed in bags in a pile on the floor. Staff reported there was no place for children to hang coats of other personal belongings. 2. There was no individual space for children's personal belonging in the toddler classroom. Children's belongings (backpacks) were observed in on the floor. Plan of Correction: Per the owner: I will get new hooks installed in both classrooms to ensure space for each child's belongings.
Standard #: 22VAC40-185-560-G Description: Based on observation, the facility did not ensure when food is brought from home, the food container are clearly dated and labeled in a way that identifies the owner. Evidence: 1. Three out of four lunch boxes in the three year old room were not labeled with today's date. 2. Two our of three lunch boxes in the two year old room were not labeled with today's date. Plan of Correction: Per the owner: We are making labels that will stay on each lunch bag permanently that the date can be written on daily. Until the labels are done we will be sure each has tape on them with the date.
