Technical Assistance:
Consultation was given on new CPS format letters, playground mulch, indoor equipment and staff training opportunities.

An unannounced renewal study was conducted today from 10:00-2:50. All rooms were in ratio with direct care staff. Observed children in classrooms in free play, snack, rest time, preparation for lunch, playground active play and centers. Reviewed a sample of staff records, children's files, medication authorizations and storage, emergency preparation and trainings. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-160-A
Description: Based on center administration statement, independent contractors do not have TB documentation. Evidence: Contractors from Fantastic Fitness, Foreign Language, Soccer and Music Enrichment as well as the independent contractor for a child does not have TB documentation on-site.

Plan of Correction: Contractors will be notified and screening will be requested.

Description: Based on record review, 2 of 4 staff records did not have documentation of receiving child abuse/mandated reporting by the end of the first day of assuming job responsibilities. Evidence: Staff # 1 (date of hire 2/1/2/2018) and staff #3 (date of hire 4/16/2018) did not have the documentation of obtaining the VA DSS child abuse and neglect training.

Plan of Correction: Certificates will be printed out and placed in staff record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the playground and center are not maintained in a clean, safe or operable condition. Evidence: 1. The small sofa is broken in the Hawaii classroom 2. The ceiling tile was not properly installed in the NY Harbor boys bathroom 3. The large wooden cabin has wall board that is falling apart and exposed nails along the window frame. 4. The fence in the small playground has an area that the wood is warped and one exposed nail is pushing up. 5. A nail is exposed on the small playground gate. 6. The rug was frayed on one corner in the Hawaii classroom 7. Glass jars and a box of gloves in the Olympic Classroom. 8. Broken water fountain on the playground that is blocked and has standing water.

Plan of Correction: Areas will be repaired or removed.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the minimum of 6 inches of resilient surface material (mulch) was not present under the two swings on the large playground. Evidence: The two swings do not have 6 inches under the area where the children's feet and weather have pushed the mulch away.

Plan of Correction: Mulch will be raked and maintained under the swings.

Description: Based on observation, posted maps do not contain all required information. Evidence: Maps posted throughout the center did not designate a primary, secondary and shelter-in place location.

Plan of Correction: Maps were redone and posted during the inspection.

Description: Based on observation, two lunch boxes were not labeled properly. Evidence: Two lunch boxes in the cubby area from the Kindergarten class were not labeled with the child's name and date.

Plan of Correction: Parents will be notified today to label and date lunch boxes.
