22VAC40-191 Background Checks (22VAC40-191)

Today's visit was a routine monitoring inspection. The inspection began at 11:00am and concluded at 3:30pm. There were nineteen children present between the ages of two and five years with five staff members, and thirty-two school age children present with five staff. During the inspection lunch time was observed, nap time was observed, required postings were viewed, emergency supplies were viewed, five medications along with authorizations were viewed, arrival of school age children was observed, one van used for transportation was viewed, seven out of sixty-five children's records were reviewed, and four out of fourteen staff records were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on the inspector's review of seven out of sixty-five children's records, a portion of the sample did not have all of the required information. Evidence #1: The record for child #1 does not have the physical address for one out of two required emergency contacts.

Plan of Correction: The PD will obtain and add the information to the children's records.

Standard #: 22VAC40-185-420-E-3
Description: Based on the inspector's review of seven out of sixty-five children's records, a record did not have confirmation from the parent at least annually that the required information for the child was up to date. Evidence: The record for child #2 has not had confirmation from the child's parent that the required information was up to date since 9/30/16.

Plan of Correction: The parent will be asked to review the child's record and update if necessary. The review will be documented.

Standard #: 22VAC40-185-450-D
Description: The inspector observed a total of nineteen children during nap time, there were ten children with pillows that did not have pillow cases and were not assigned for individual use. Evidence: The inspector observed a total of nineteen children during nap/rest time. There were ten children with pillows that did not have covers and the pillows were not in any way assigned for individual use.

Plan of Correction: The pillows will be labeled in a manner to ensure individual use and each pillow will be provided with a removable case.

Standard #: 22VAC40-185-510-C
Description: Based on the inspector's review of four medications, the center did not have duration of parent authorization for the administration of the medication per policy. Evidence: The medication authorization forms for two different medications for child #1 did not include the duration or length of time of the parent's authorization to administer the medication.

Plan of Correction: The Program Leader for the school age program will have the parent put a duration on the form.

Standard #: 22VAC40-185-510-G
Description: Based on the inspector's review of five medications, a prescription medication was not maintained in the original container with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: The inspector observed an Epi-Pen in a ziploc bag and was no longer in it's original box. The Epi-Pen had a prescription label and no instructions for use.

Plan of Correction: The original box will be requested by the facility.
