Evidence: The record for child #2 has not had confirmation from the child's parent that the required information was up to date since 9/30/16. Plan of Correction: The parent will be asked to review the child's record and update if necessary. The review will be documented.

Standard #: 22VAC40-185-510-C Description: Based on the inspector's review of four medications, the center did not have duration of parent authorization for the administration of the medication per policy. Evidence: The medication authorization forms for two different medications for child #1 did not include the duration or length of time of the parent's authorization to administer the medication. Plan of Correction: The Program Leader for the school age program will have the parent put a duration on the form.
Standard #: 22VAC40-185-510-G Description: Based on the inspector's review of five medications, a prescription medication was not maintained in the original container with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: The inspector observed an Epi-Pen in a ziploc bag and was no longer in it's original box. The Epi-Pen had a prescription label and no instructions for use. Plan of Correction: The original box will be requested by the facility.
