An unannounced monitoring inspection was completed today. This is a 2 month follow-up visit from the facility having been placed on a Provisional license on 7/23/18. Areas of the center, 6 staff files, 3 children's files, and emergency procedures were inspected between 1:00-2:20pm. There are 90 children under the supervision of 25 staff. For lunch, the children were served beef stir-fry with mixed veggies, rice, pears and milk. Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-185-60-A Description: Based on file review, 1 out of 5 children's records do not have all required information. Evidence 1. Child #5 does not have addresses for the two emergency contacts, in case the parents cannot be reached. Plan of Correction: The Director will ask the parents for this. They will also edit the form that is completed annually by the parent to include this information.
Standard #: 22VAC40-191-40-D-6 Description: Based on file review, the center accepted a central registry finding (CRF) for an employee that was dated more than 90 days prior to the date of employment. Evidence 1. Staff #6 had been hired at the center on 11/18/13 and resigned on 11/11/16. This staff was then rehired on 7/17/18. The only CRF on file was completed on 2/6/14, which was more than 90 days prior to the date of hire. The director had thought this was effective for 5 years, but as a rehire, this must be completed again if the above time frame cannot be applied. Plan of Correction: The Director will complete this again.
