Comments:
A Subsidy Health and Safety monitoring inspection was conducted on 9/7/2018 to review supplemental health and safety requirements. This inspection began at 2:00pm and ended at 3:30pm. There were 2 staff with 29 children in the school cafeteria at the time of the visit. At the start of the inspection, children were observed arriving from school, checking in with the CASA staff and playing freely in small groups. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were no medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Description: Based on a review of medication authorizations and interview, the program has not obtained a written allergy care plan for one child with a diagnosed food allergy. Evidence: Child #6 has emergency medication kept on-site in case of accidental ingestion of an allergen. There is no documentation of an allergy action plan which states the steps to be taken in the event of an allergic reaction. Staff #1 has confirmed that they do not currently have the allergy care plan and that they are working on obtaining one from the parents.

Plan of Correction: I'll ask the parents again today to get a copy from the school so that they can provide us with a copy on Monday.
