Technical Assistance:
Licensing inspector discussed 70-A (files of new staff) and 40-J (injury prevention procedures) with director.

Comments:
An unannounced renewal inspection was conducted today from approximately 9:45am-3:35pm. There were 67 children in the direct care and supervision of 10 staff. A sample of 7 children?s records, 5 staff records and 4 medications were reviewed. The children were observed playing, eating snack, learning about caterpillars, painting, and engaged with staff. Licensing inspector noticed excellent classroom management skills displayed. One new staff member has been hired since the last inspection. Areas of non-compliance are identified in this report. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation the each child has received immunization. Evidence: Child F did not have documentation of immunization on file.

Plan of Correction: I will have the child's parent bring in required paperwork by 9/14/2017.

Standard #: 22VAC40-185-60-A
Description: Based on records reviewed, children's records were incomplete. Evidence: 7 of 7 children's records reviewed were missing required information. 1) Children A-G did not have a space in their file for the parent to list a nickname if applicable. 2) Children A-G did not have the first day of attendance noted in their file. 3) Children B,D and G did not have 2 of 2 required addresses for the emergency contacts. 4) Child C did not have the residential addresses of both parents on file.

Plan of Correction: Required information will be added to files by Friday.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: 3 of 5 staff records were incomplete. 1) Staff B was missing 2 of 2 references, orientation and proof of playground training. 2) Staff C was missing orientation, and proof of playground training. 3) Staff D was missing date of hire, diploma or transcript, 2 of 2 references, and orientation.

Plan of Correction: All staff files will be updated tomorrow.

Description: Based on observation, the asbestos plan was not followed. Evidence: The asbestos plan on site expired 07/31/2013.

Description: Based on observation, a notice regarding asbestos was not posted at the location. Evidence: The asbestos plan for the center was not posted. The plan was located off site and was delivered in the middle of the inspection.

Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: Cleaning materials and disinfectant wipes were found located in an unlocked cabinet above the sink in the classroom of Staff A.

Description: Based on observation, electrical outlets were not protected with covers. Evidence: 1) 1 electrical outlet was uncovered on the wall near the first classroom. 2) 1 electrical outlet was uncovered on the wall below the silver clock. 3) 2 electrical outlets were uncovered on the wall near the aftercare area.

Plan of Correction: The covers are being ordered. Outlets will be covered when they arrive.

Standard #: 22VAC40-185-510-C
Description: Based on record review, the medication authorization plan on file was expired. Evidence: The medication authorization plan for Child E expired 10/13/2017.

Plan of Correction: I can ask her parents for the form but it requires a doctors signature. The form will be on file by 9/14/2018.

Standard #: 22VAC40-185-550-D
Description: Based on record review, evacuation drills were not practiced monthly. Evidence: Evacuation drills for the months of May, June, July and August were not practiced.

Plan of Correction: September fire drill will be conducted on Tuesday.

Standard #: 22VAC40-185-550-M
Description: Based on record review, minor injury records were incomplete. Evidence: 1 of 1 minor injury record reviewed was missing the following information. 1) Method of parent notification. 2) Time and date in which parents were notified. 3) Future action to prevent injury.

Plan of Correction: Both her parents are out of town they will be back on Friday.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food containers were not clearly labeled with the name of the child and date. Evidence: 15 Lunch bags located near the classroom of Staff A, were not clearly labeled with the name of the child and date.
