We discussed the requirements for medication training and staff coverage, menu availability, and staff records. The risk assessments for violations were provided. During the inspection, we talked about options for the routine, program activities and space use at the school during the first forty-five minutes after school dismissal. Children's record requirements were reviewed. In addition, we reviewed the requirements for sight and sound supervision during transitions in the hallways.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 2:15 PM to 6:20 PM. Today, there were thirty-eight children after school with three to four staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, posted information, menu, nutrition, medication, emergency supplies and drill records, five records for children, seven staff records, staff qualifications, staff training, and indoor areas. Homework assistance was provided in a classroom. The children were provided table activities and games. Staff were seen providing frequent interactions with the children and appropriate behavioral guidance. Standard #: 22VAC40-185-70-A Description: Based on a review of six records for staff, and interviews with staff, there was not any written information to demonstrate that staff member 1 possesses the education required for the job position held. Evidence: Staff member 1 began work at this center August 13, 2018 as a group leader on August 13, 2018 after working as a leader at a different location since January 18, 2018. A high school diploma or education transcripts were not in the staff file. Plan of Correction: Administrative staff will obtain a copy of the high school diploma or transcripts from staff member 1 and place the information in the staff record. In the future, all required education information will be obtained at the time of promotion or job assignment.
Standard #: 22VAC40-185-240-D-4 Description: Based on the review of medication prescribed and present for an attending child, a review of staff training records, and interviews with staff, child 1M was not always in the care of a staff member who met the requirements for medication administration training. Evidence: There was an emergency medication kept at the center for a diagnosed need. The children arrived after school beginning at 3:18 PM. Staff member 2 was the only afternoon staff member with the required specified training in addition to M.A.T. and regularly does not arrive until 4:00 PM or later. Today, staff member 2 arrived at 4:05 PM. Plan of Correction: Arrangements were immediately made for staff member 1 to complete the training. The on-line course required for the specified emergency medication was completed on September 6, 2018. In the future, administrative staff will make sure there are trained staff present whenever a child has been prescribed emergency medication that requires additional training according to the approved M.A.T. curriculum.
Standard #: 22VAC40-185-560-F Description: Based on a review of the posted information, and interviews with staff, a menu listing foods to be served for meals and snacks during the current one-week period was not posted or given to parents. Evidence: The snack menu for the month of August was on the bulletin board in the hall outside of the cafeteria. Staff indicated that the menu is kept there and they could not locate the current menu. A staff member brought a box of food items from the main office close to 4:00 PM and a menu was inside of the box. Plan of Correction: The menu was taken from the snack box and posted on the bulletin board in place of the August menu. The site program leader/director will always make sure a current menu is posted in view on the hall bulletin board.
