63.2 Licensure and Registration Procedures

Technical Assistance:
The license cannot be processed for renewal until all required background checks have been viewed by the Inspector. The license expiration date will be revised in order to better coordinate with the school calendar and inspection timeline requirements.

Comments:
An unannounced full renewal inspection was conducted today. There were 84 children in care, with a total of 14 staff members assigned to their direct supervision. Children's records, background checks, staff training, staff records, children's medications, administrative documents, emergency preparedness items, and the physical plant were inspected today. The children were observed happily playing outside in a light mist rain, creating theme based art projects, enjoying group discussions, eating snack from home, and interacting with staff members. Mid day dismissal/arrival procedures were also observed today. This inspection was completed between approximately 10:30am and 12:45pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-80-A
Description: Based on a review of administrative documents, it was determined that one classroom has not completed their daily written record of children in attendance. Evidence: Today's written attendance was not completed in the Foxes classroom.

Plan of Correction: Staff documented the attendance immediately during the inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that one potentially hazardous substance was not stored in a locked place. Evidence: In the Owls bathroom, there is a spray bottle of cleaning solution stored on the counter next to the diaper changing pad. This cleaner is used to spray down the inside of the diaper trash can.

Plan of Correction: The cleaner will be moved to a locking cabinet.

Standard #: 63.2-1720.1-B-3
Description: Based on a review of staff background checks, it was determined that two required out of state background checks were not requested. Evidence: Staff B and Board Member C have both resided outside of the state of VA within the past 5 years, as is documented on their Sworn Disclosure Statements. Management staff have not submitted the out of state Central Registry search requests for each of these individuals.

Plan of Correction: Both out of state search requests will be submitted immediately. The documentation of each search request will be forwarded to the Inspector within the next 10 days.
