An unannounced monitoring inspection was conducted today from approximately 3:30pm-6pm. There were 112 children in the direct care and supervision of 10 staff. A sample of 10 children?s records, 10 staff records and 5 medications were reviewed. The children were observed playing on stage, playing in the gymnasium, crafting in the cafeteria, during transition, and during dismissal. The staff did a great job at meeting the children where they were in relation to their age and stage. Approximately 5 new staff were new to this location since the last visit. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: 1 of 10 records were missing required information. 1) The file of Child A was missing the residential information and the employment information of the custodial father. Plan of Correction: Child A has proof of birth certificate information in the file.
Standard #: 22VAC40-185-260-A Description: Based on observation, the center did not have an annual fire inspection conducted. Evidence: The last fire inspection expired 9/1/2018 Plan of Correction: Company will work with school personnel to ensure we receive up to date fire inspections.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: A can of spray glaze labeled "keep ou of reach of children" was found unlocked in a cabinet located in the Pod area closest to the entrance of the school. Plan of Correction: Corrected during inspection.
