A Subsidy Health and Safety inspection was conducted on 09/06/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 3:30pm-6pm. There were 112 children in the direct care and supervision of 10 staff. A sample of 10 children?s records, 10 staff records and 5 medications were reviewed. The children were observed playing on stage, playing in the gymnasium, crafting in the cafeteria, during transition, and during dismissal. The staff did a great job at meeting the children where they were in relation to their age and stage. Approximately 5 new staff were new to this location since the last visit. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-G Description: Based on record review, first aid and CPR training was not maintained by each staff. Evidence: 8 of 10 staff members had expired First Aid and CPR training. 1) The First Aid and CPR trainings of Staff A-H expired 8/2018. Plan of Correction: Management will ensure all staff first aid and cpr cards are on site and up to date.
