Comments:
An unannounced monitoring inspection was conducted today from9:30am-12:30pm. There were 68 children supervised by 13 direct care staff. The physical plant, staff records, children?s records, medications, evacuation drills, programming and policies were inspected. Children were observed participating in group play, art, interactive electronics, morning circle time, reading books, playing outside, lunch and getting ready for nap time. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center is not maintained in a clean, safe and operable condition. Evidence: In the Creative Arts room, a broken blind was observed, with a slat from the blind hanging down. In the same room, the screen is ripped leaving a cord accessible to children causing a potential head/neck entrapment Chipped paint was observed in the Creative Arts and Language Arts classrooms. The fence on the playground to the far right has a broken picket, leaving the bottom 6 inches open. The gate to the parking lot on the playground to the far right is broken, leaving the parking lot accessible to children. The playground in the middle has a fenced area, that has a lot of trash thrown in it. The playground to the far left, has a 2"-3" hole, that is approximately 2" deep, causing a potential tripping hazard.

Plan of Correction: I will put in a maintainance ticket today to get everything fixed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, potentially hazardous substances such as cleaning agents and "keep out of reach of children" items were stored in an area that was not inaccessible to children. Evidence: Unlocked cabinet's and doors were observed in the Language Arts room. leaving bleach, suntan lotion and hazardous office supplies accessible to children.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observations, the center does not have a hand free system in place to dispose of soiled diapers. Evidence: The diaper disposal garbage can in the Language Arts classroom, located next to the changing table has a broken foot pedal, making it difficult to pick up the lid without using your hand.

Plan of Correction: I will order a new trash can today, it should arrive within 2 weeks.

Standard #: 22VAC40-185-510-G
Description: Based on review of two medications, the center did not have current written authorization to administer medication to children in care. Evidence: Based on review of medications, the center has expired medication authorizations for child #1, authorization expired 8/7/18 and child #2, authorization expired 5/30/18.

Plan of Correction: I will contact the parents involved and get new authorizations.

Standard #: 22VAC40-185-560-G
Description: Based on observation, when food is brought from home it is not being labeled with name and date. Evidence: In the Language Arts classroom, lunch boxes from home are not labeled with name and date, and nutritional drinks in the refrigerator were not labeled with name and date. Observed in the Creative Arts classroom, was an unlabeled lunch box from home.

Plan of Correction: I will send out an email to all staff reminding them that everything must be labeled with a name and date.
