22VAC40-185 PHYSICAL PLANT.

Had a discussion with the center owner regarding replacing the straps on the infant buggy, as several of them are broken or missing as well as the blankets, pillows and toys located in two of the children's cribs.
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 8/30/2018. Observations were made regarding the upkeep and maintenance of the physical plant. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Standard #: 22VAC40-185-270-A Complaint related: Yes Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition. Evidence: 1. There is a metal nail sticking through the fence of one playground with the back half of the nail protruding onto the older playground. 2. The gate outside to exit the toddler playground does not open. 3. The foot pedal of the trash can in the two's classroom is broken. There is a piece of metal protruding out of the trash can from where the pedal used to be. 4. The laundry room walls are covered in clumps of lint from the dryer. The dryer was running during the time of the inspection and a large vent has a gap releasing hot air into the laundry room causing the room temperature to rise dramatically. 5. There is a large cut out in the metal at the bottom of on of the AC units. The bottom of the AC unit contains approximately an inch of sitting water. 6. In a storage cabinet on the back playground there are several birds nests on the top shelf. There is a dead bird located at the bottom of the storage cabinet. 7. On the back playground, there is a trike with broken foot pedals and a broken handle, a ladder laying on the ground, a picnic table flipped on it's side and a large shovel and pitchfork laying on the ground. The center owner has stated that this playground is not used by the children, however the latch on the gate to the playground is broken, making all of these materials accessible to the children. Plan of Correction: The nail has been hammered down. The trash can has been thrown away. A plumber is scheduled to come look at the AC unit. Maintenance requests have been filled out for the rest of the items to be fixed.
Standard #: 22VAC40-185-280-B Complaint related: Yes Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: A large closet/laundry room labeled "Staff Only" contains various paint cans, weed killer, ice melt and several cleaning agents. There is a lock at the top of the door, however at the time of the inspection the door was unlocked and open, making all of the hazardous substances accessible to the children. Plan of Correction: I will put a sign up on the door that the door needs to stay closed. Maybe I'll move all the chemicals to another cabinet with a lock on it.
