Technical Assistance:
Consultation was given on disposing medications and staff records.

Comments:
An unannounced monitoring visit was conducted today from 9:45-10:45. Nine children were present with two staff and within ratios for supervision. Room is well equipped with age appropriate materials and equipment. No medication is on-site today. Observed children participating in coloring, blocks, house keeping play area and transition to the playground. Reviewed 3 children's records and emergency preparation. . Standard #: 22VAC40-185-70-A
Description: Based on center administration statement, 2 staff records that are kept on-site are incomplete. Evidence: Emergency contacts for both staff members are not at the center.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-280-B
Description: Based on observation, a cabinet in the bathroom was not locked and contained a hazardous substance. Evidence: A bottle of bleach was in the bathroom cabinet and the safety lock was not secured.

Plan of Correction: The cabinet was locked during the inspection.

Standard #: 22VAC40-185-510-N
Description: Based on staff statement, a medication was not disposed of by the center after child was no longer enrolled. Evidence: An emergency medication was in the first aid box. Staff stated that the child was no longer enrolled after 5/2018 and the parents had been notified to pick up medication. The parent said it was expired and they would not be picking it.

Plan of Correction: The medication will be taken to a pharmacy or given to Parks and Recreation administration for proper disposal.

Standard #: 22VAC40-185-560-G
Description: Based on observation, lunch boxes are not labeled with name and date. Evidence: Based on a random sample of lunch boxes, the date was missing on 5 lunch boxes and names on 3 lunch boxes.

Plan of Correction: Center staff will remind parents to use stickers by the sign in sheet to label and date lunch boxes each day.

Description: Based on observation, the most recent inspection notes are not posted. Evidence: The Inspection notes posted are 9/13/2017 and the most recent inspection notes are 1/30/2018.

Plan of Correction: The new notes will be posted.
