Comments:
An unannounced complaint inspection was conducted on 09/05/2018 in response to a complaint that was received regarding the care and safety of children and documentation of children's injuries. The investigation included staff interviews, a review of records and injury reports as well as a review of the outdoor play area. The allegation regarding the care and safety of children was found to be not valid. One injury was not documented by center staff therefore, this area of the complaint is found to be valid. The violation for injury documentation is listed on the violation notice and was discussed with center staff at the exit interview conducted by telephone on 09/11/2018.

Description: Based on record review and interview, the center failed to maintain a written record of a child's minor injury in which entries are made the day of occurrence. Evidence: 1. Child 1 received a small bruise to the upper eye lid/ eye brow area in July of 2018 while in attendance at the summer camp. a. Staff 1 stated that the injury had not been noticed by staff as child 1's hair covered the area. b. The injury was brought to the attention of staff 1 by the parent of child 1. 2. Staff 1 stated that she investigated this injury and was able to determine that child 1 was injured when another child hit her in the face with his jacket. 3. Staff 1 confirmed that she failed to write an injury report for this injury.

Plan of Correction: Program Director and Child Care Director have discussed what incidents should be documented and moving forward all incidents will be documented fully. Child Care Director will inform staff of the types of incidents that should be documented.
