22VAC40-191 Background Checks (22VAC40-191)
An unannounced inspection was completed on 09/18/18 from 10:15 A.M.-1:30 P.M. At the time of the inspection 57 children were in care with eight staff and two administrators present. Five children records, five staff records, four medications, five injury reports, evacuation drills, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during small group activities, fine motor activities, painting, music, snack time, handwashing, lunchtime, and naptime. Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to ensure that all outlets had a protective cover. Evidence: 1. In the second primary classroom behind the fish tank there was a power strip that did not have a protective cover. 2. Staff verified that the outlet did not have a protective covering. Plan of Correction: The administrator will get a protective cover from their storage unit and place in the outlet, and remind staff the importance of checking all outlets at the beginning of each day.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the center failed to ensure that all required components were on the written injury reports. Evidence: 1. One out of five injury reports was missing the time of day that the injury took place. 2. One out of five injury reports was missing the future action to prevent recurrence of the injury. 3. Staff verified that the reports were missing the components. Plan of Correction: The administrator will provide a training to all staff on the proper way to complete the Injury Reports.
