An unannounced inspection was conducted today, June 27, 2018 in response to a complaint received by The Department of Social Service, Children's Licensing on June 20, 2018 concerning preventing the spread of disease and parent notification of a serious injury. The inspection was initiated at 12:30 pm and concluded at 2:05 pm. Interviews were conducted with staff, four children's records were reviewed, and documentation of the center's illness policy and injury report reviewed. An observation was made in the school age building and measurements were taken of the resilient surfacing on the playground. Based on the information gathered during the investigation, the preponderance of evidence supports the allegation regarding parent notification of a serious injury, therefore the complaint is valid. An additional violation was cited based on observation regarding ratios that was not related to the complaint. Description: Based on observation, the facility did not ensure for children, from age of eligibility to attend public school through eight years, there was one staff member for every 18 children. Evidence: In the school age building, twenty four children were observed with one staff.

Plan of Correction: Corrected, Per the administrator: secondary teacher was present and immediately sent back to class

Description: Based on interview, the facility did not ensure parents were notified immediately when a child required emergency medical treatment or sustained a serious injury. Evidence: 1. A serious Injury was documented on June 22, 2018 at 4:09 pm for child #1. 2. Staff #1 reported she called parent #1 multiple times at 4:20 pm, however the parent did not answer. 3. Staff #1 reported she did not attempt to call parent #2. 4. Staff #1 reported she did not attempt to call either of the two emergency contacts. The child's record indicted to call the grandmother if parent could not be reached. 5. Staff #2 drove child #1 home, arriving at approximately 4:45 pm, and notified parent #1 of the injury, recommending that the parent seek medical attention. 6. Child #1 was seen by a physician and a diagnosis of a broken wrist was given.

Plan of Correction: Per the Administration: GLC staff will continue to reach out to parents first when emergencies arise. If contact isn't made with each attempt GLC staff will call emergency contacts. GLC staff did and will continue to document all injuries and provide the necessary first aid care to all students in our care.
