Comments:
Licensing inspector arrived at 3:54pm and departed at 5:30pm to conduct an unannounced monitoring inspection. There were 16 school age children present with two staff members. An additional staff person arrived throughout the course of the inspection for administrative and support purposes. The children were observed playing with a variety of table top activities and interacting with staff and each other. The posted menu indicated that the evening snack would be cereal and milk. Five children's records were reviewed during this inspection. Staff indicated that there was no medication at the center. The outdoor play area was not inspected due to inclement weather.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in one of five children's recors Reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on September 18, 2018, there was no documentation of one parent's address, place of employment, phone number and work phone number for Child #1.

Plan of Correction: I will have the parent to fill out the form for that parent's information or have the parent to fill out N/A.

Standard #: 22VAC40-185-70-A
Description: Based on record review, in one of three staff records reviewed, the licensee did not ensure that all required information was staff was kept at the center. Evidence: During the inspection conducted on September 18, 2018, there was no documentation of the staff name, address, verification of age requirement, job title, date of employment and the name, address and telephone number of a person to be notified in an emergency for Staff #2.

Plan of Correction: Will talk to the Area Manager to get the paperwork to put into the licensing book.

Standard #: 22VAC40-185-240-D-5
Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: During the inspection conducted on September 18, 2018, upon arrival to the facility at 3:54pm, there were two staff members present and neither staff member had documentation of current DHO training. Throughout the course of the inspection, Staff #3 arrived at 4:30pm with current DHO training.

Plan of Correction: Violation corrected while the inspector was present.

Standard #: 22VAC40-185-260-A
Description: Based on observation and inspection of the facility, the licensee did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on September 18, 2018, the fire inspection report for the facility was more than a year old and was dated 4/6/17.

Plan of Correction: The Area Manager has been informed about the ipaperwork and is awaiting for an inspection to be completed.
