Comments:
An unannounced monitoring inspection was conducted on September 19, 2018. There were 39 children present at arrival to the facility with four staff members. An additional staff person arrived for support purposes. The children were observed playing in centers and transitioning to outdoor play. Two medications were reviewed in the facility on this date. Five children's records were reviewed. A large open field is being used for the outdoor play area, as the program is no longer utilizing the playground equipment at the school.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on September 19, 2018, the following information had not been documented in each child's record: Child #5-There was no documentation of the place of employment and work phone number for one parent.

Plan of Correction: We will contact the child's parent and update the record.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the staff members did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on September 19, 2018, the following unlocked hazardous substances were observed in Classroom 2: 1. There was a bottle of bleach-water solution on the counter top next to the sink children use to wash their hands. 2. The safety latch was not closed on the upper cabinet and there were three bottles of bleach, one can of disinfectant spray and one container of disinfectant wipes.

Plan of Correction: The cabinet was locked during the inspection and the bleach-water solution was locked also.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation, the center did not ensure that a monthly practice evacuation drill was conducted. Evidence: During the inspection conducted on September 18, 2018, the center staff did not provide documentation of an evacuation drill being practiced in June 2018.

Plan of Correction: We will look for the fire drill documentation and post it in the classroom.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on September 18, 2018, the facility had findings of an inspection conducted on March 15, 2018 posted but the most recent inspection for the facility was conducted on June 13, 2018.

Plan of Correction: The inspection from today will be posted.
