The licensing inspector conducted an unannounced monitoring inspection Thursday, September 13, 2018 from 10:50am to 1:45pm. The infants were observed playing in exersaucers and being fed their lunch. The older infants and toddlers were observed eating lunch. Lunch today consisted of corn dog nuggets, mashed potatoes, mixed fruit and milk. The menu was posted. All classrooms and playgrounds were inspected today. Five children?s records and five employee records were reviewed during this inspection. The center administers medication, however none is being administered at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 08/21/2018 Shelter-in-place drills: 03/06/2018 & 08/3/2018 Last fire inspection: 06/25/2018 Last health inspection: 06/29/2018 Total enrollment today: 33 Today, the following child to staff ratios were observed: Infant I (6 weeks to 12 months) ?9:3 Infant II (12-14 months) ?7:2 Toddler I (14-18 months)-9:3 Toddler II (18-24 months)- 8:3 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records on 09/13/2018, the center did not ensure that two of the five records reviewed contained the required information. EVIDENCE: 1. The record of Child #1 did not contain a phone number for the second emergency contact listed. 2. The most recent documentation of a review of the Child #2's emergency contact information by the child's parent was when the child started at the center on 01/23/2017. Plan of Correction: 1. Going forward will make sure to get the phone number for all emergency contacts. 2. Will get the child's mom to update the information today
Standard #: 22VAC40-185-270-A Description: Based on observation on 09/13/2018, the center did not ensure that areas of the center shall be maintained in a clean, safe and operable condition. EVIDENCE: 1. In the Infant II and the Toddler II room there were several areas of the walls where the paint had chipped off. 2. There was a buildup of dirt around the base of the toilets in the Infant II, Toddler I and Toddler II bathrooms. Plan of Correction: 1. Painting will be scheduled by end of month. 2. Will have cleaned today.
Standard #: 22VAC40-185-280-B Description: Based on observation on 09/13/2018, the center did not ensure that hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: A spray bottle labeled "bleach and water" was observed in an unlocked cabinet on the school age room. Plan of Correction: Corrected on site. Will get a new cabinet to lock the hazardous substances in.
Standard #: 22VAC40-185-520-D Description: Based on observation on 09/13/2018, the center did not ensure that insect repellent was labeled with the child's name. EVIDENCE: Three unlabeled bottles of insect repellent was observed in the third bathroom in the older infant/toddler side. Plan of Correction: Corrected on site. Staff labeled containers.
