Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on August 28, 2018 regarding allegations of supervision and infant feeding. The inspection was completed on September 13, 2018. The inspector was on site from 10:50am to 1:45pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports parts of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. If you have any questions about this inspection, please contact the licensing inspector at (804) 662-9760. **PROVIDER** Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within five business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of documentation on 08/13/2018, the center did not ensure that infants were fed in accordance with parental instructions. EVIDENCE: 1. The infant feeding schedule for Child #1 states that Child #1 should have 6 oz. of formula every 2 hours. 2. The 8/8/2018 daily parent report for Child #1 documented a 4 oz bottle at 10am and another 4 oz bottle at 4:15pm 3. The 8/9/2018 daily parent report for Child #1 documented a 4 oz bottle at 10am and another bottle at 4:30pm
