Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:00am - 12:15pm. There were fifteen preschool children in care with three staff and one volunteer. The children were observed playing in centers, at story time and during circle time. Five children's records were reviewed. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the outside play area in a safe condition. Evidence: 1. The inspector observed the outside play area. There is a broken landscape timber with sharp edges. The timber surrounds the area where the climbing equipment is located. There is hole next to the basketball court. The hole is large enough for a child's foot.

Plan of Correction: The teacher will complete a maintenance request today and ask that the landscape timber be replaced. The teacher filled the hole with gravels during the inspection.

Description: Based on observation, the center failed to have protective covers in all outlets. Evidence: 1. The inspector observed two outlets without protective covers on the powerstrip next to the teacher's desk.

Plan of Correction: The teacher placed protective covers in the powerstrip during the inspection.
