22VAC40-191 Background Checks (22VAC40-191)
- Technical Assistance:
Discussion of the newly obtained finger print checks. These national background checks replace the criminal record checks done through the state and are good for 5 years. In addition, the existing search of the central registry (CPS) checks and Sworn Disclosure Statements (SDS) are retroactively good for 5 years from the date of finding (for CPS) or completion (for SDS). As a reminder, new employees will not be allowed to start until an ?Eligibility Letter? is on file, and any new staff person who lives or has lived outside of Virginia within the past 5 years will need to have an out-of-state CPS check requested to be placed in their file as well.
The following inspection took place between the hours of 9:45 am through 3:30 pm. Today, I met with the Staff of the Goddard School located in Leesburg, Virginia. At the start of this unannounced renewal inspection there were a total of 95 children with 17 classroom staff; good ratios in all 9 rooms. A total of 10 children?s files and 8 staff files were reviewed. The children were engaged in the following: large motor skills outside on center playground, art, games, music, story time, diapering, bathroom breaks, hand washing, center activities within the classroom, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-160-C Description: Based on review, 1 of 8 staff files reviewed indicated that it has been more than 2 years since the most recent tuberculosis (TB) documentation of file. Evidence - 1. On the date of inspection (09/21/2018), the most current TB documentation on file for Staff #4 was dated - 03/14/2016. 2. This documentation was observed within the staff file of Staff #4. Plan of Correction: Updated documentation shall be obtained.
Standard #: 22VAC40-185-60-A Description: Based on review, 2 of 10 children's files reviewed indicated that not all of the required documentation was contained within each child's record. Evidence - 1. On the date of inspection (09/21/2018), 1 of 2 emergency contacts were listed for Child #2, and 0 of 2 emergency contacts were listed for Child #9. 2. The Health forms (physical & immunization page) were not available for review for Child #2. The start date for Child #2 was listed as 10/11/2017. Plan of Correction: All missing information/documentation shall be obtain from each applicable child's parent and then placed on file.
Standard #: 22VAC40-185-70-A Description: Based on review, 2 of 8 staff files reviewed did not contain all of the required documentation. Evidence - 1. On the date of inspection (09/21/2018), 2 references for the following staff were unavailable for review: Staff #2, and Staff #3. 2. Both staff were hired after March 1, 1996. Plan of Correction: These references shall be found and/or, if needed, re-checked and placed on file.
Standard #: 22VAC40-185-280-B Description: Based on observation and interview, a hazardous substance (cleaning materials) was not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (09/21/2018), within the Pre-Toddler room, a spray bottle containing clear liquid was observed not in use, stored on a shelf. 2. A staff person within this room stated (during an interview conducted on the date of inspection) that the spray bottle contained a bleach/water solution. Plan of Correction: This container shall be placed in a locked area.
Standard #: 63.2-1701-G Description: Based on observation, the most recent issued violation notice was not posted. Evidence - 1. On the date of inspection (09/21/2018), an inspection summary for a SHSI inspection was observed posted in a frame. 2. The Violation Notice issued on the same date of -3/06/2018 for an Unannounced Monitoring Inspection (UMI) was not. 3. The Violation Notice was kept in a binder within the center office. Plan of Correction: The Violation Notice issued today shall be placed in a sleeve in order to view all current reports and posted.
