Comments:
An unannounced renewal inspection was conducted on 9/11/18 from 10:30 am until 1:00 pm. At the time of the inspection 40 children were in care with seven staff. The sample size consisted of five children's records, six staff's records and three board officer's (agents) records. Children and staff were observed during outdoor play, hand washing, lunch, nap preparations, transitions, and behavioral guidance. Standard #: 22VAC40-185-210-A
Description: Based on record review and interview, staff 6 does not meet one of the qualifications to be a lead teacher. Evidence: 1. The record for staff 6 documents the staff title as lead. Documentation in the record shows 15 years of experience and a BA in Literature. The college transcripts are not in the record. 2. The director was unsure how staff 6 qualifies.

Plan of Correction: Staff 6 is not serving as a lead in our aftercare program. She is an assistant teacher and her records will be corrected to show this job title.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on record review and interview, the center failed to obtain a search of the central registry completed before the end of 30 days after a change of an agent . Evidence: 1. The record for agent 1 shows the date the agent was appointed as 4/23/18. The record does not contain a search of the central registry. 2. The director verified they had not completed the search of the central registry for agent 1.

Plan of Correction: The Board of Trustees bylaws will be amended to specify the requirements of holding office. Officers will be required to submit all licensing requirements prior to being sworn in.

Standard #: 22VAC40-191-60-B
Description: Based on record review and interview, the center failed to have a staff member complete a sworn statement prior to employment. Evidence: 1. The record for staff 6 has a start date of 8/22/18. The sworn statement in the record is dated 9/4/18. 2. The director verified the date of the sworn statement.

Plan of Correction: A comprehensive checklist will be developed for all hiring staff and HR personnel that clearly lists all requirements for work prior to employment.

Description: Based on record review and interview, the center failed to ensure staff submitted to fingerprint background checks prior to hire.

Evidence:

1. The record for staff 6 shows the date of employment as 8/22/18. The record contains a fingerprint background check dated 8/31/18.
2. The director verified the dates.

Plan of Correction: A comprehensive checklist will be developed for all hiring staff and HR personnel that lists all requirements needed before employment.

Standard #: 63.2-1721.1-B-2
Description: Based on record review and interview, the center failed to ensure agents of the license submitted to fingerprint background checks 30 days after appointment. Evidence: 1. The record for agent 1 shows the date the agent was appointed as 4/23/18. The record does not contain a fingerprint background check. 2. The record for agent 2 shows the date the agent was appointed as 4/9/18. The fingerprint background check is dated 8/3/18. 3. The record for agent 4 shows the date the agent was appointed as 3/12/18. The fingerprint background check is dated 4/19/18. 4. The director verified the dates.

Plan of Correction: The Board of Trustees bylaws will be amended to specify the requirements of holding office. Offices will be required to complete all licensing requirements prior to being sworn in moving forward.

Standard #: 63.2-1721.1-B-3
Description: Based on record review and interview, the center failed to submit a search of central registry maintained by any other state in which an agent has resided in the preceding five years for any founded complaint of child abuse or neglect. Evidence: 1. The record for agent 1 shows the date the agent was appointed as 4/23/18 and within five years prior to appointment he lived in Oregon. The record does not contain a search of central registry in Oregon. 2. The director verified a search of central registry in Oregon was not submitted.

Plan of Correction: The Board of Trustees bylaws will be amended to specify the requirements of holding office. Officers will be required to complete all licensing requirements prior to being sworn in.
