Comments:
An unannounced monitoring inspection was conducted today September 18, 2018 to determine the facilities compliance with licensing standards. The inspector arrived at 11:00 am and concluded the inspection at 1:30 pm. Upon the inspector's arrival, eighteen children were in care with two staff and an additional staff (parent advocate). Children were observed in the school cafeteria having lunch. Staff assisted the children with getting their trays to the table and opening items. Today's lunch consisted of black beans, carrots with dip, BBQ sandwiches, fruit and milk. Upon returning to the classroom children had bathroom breaks and washed their hands. Children later traced around a variety of stencils and colored pictures. The book "My School Day" was read before children prepared for nap. Staff interacted in a positive manner with children. All areas used for child care were inspected including the outdoor playground. The classroom had age and stage appropriate materials and supplies. All required information was posted to include the license, last inspection notice, daily schedule, emergency numbers, menu, evacuation procedures/maps and parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Three staff records were reviewed (see violation notice) along with five children's records. Staff have current MAT, CPR, first aid training and daily health observation training. Per the administrator, there are currently no medications being administered. An evacuation drill was completed on 09/12/2018. The two required shelter-in-place drill for the 2017-2018 year were documented on 09/29/2017 and 03/20/2018. Two shelter-in-place drills for 2018-2019 will need to be completed by the end of the school year. Fire Inspection: 08/30/2018 Health Inspection: 12/14/2017 Violations cited during the lisensure period were checked for corrections, there were no repeat violations cited during today's inspection. Standard #: 63.2-1720.1-B-3
Description: Based on review of staff records, the facility did not obtain the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: 1. The record of staff #2, hired 04/04/2018, did not have the results of an out of state central registry. 2. Documentation indicated that a request for the out of state central registry was made on 02/27/2018 however a follow up was not conducted within 45 days after the request was made.

Plan of Correction: Per the administrator: A new request will be made today and results will be obtained within the time allowance. We will send the inspector a copy of the request within 10 days.
