22VAC40-191 Background Checks (22VAC40-191)
An unannounced inspection was completed on 09/20/2018 from 11:15 A.M.-2:45 P.M. At the time of the inspection 58 children were in care with ten staff, one office coordinator, and one administrator present. Six children records, six staff records, three medications, emergency plan, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during storytime, infant feeding, diapering, handwashing, lunchtime, and naptime. The exceptions to compliance are noted on the violation notice. Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In the bathroom located near the Circus Room there was a can of N1-712 Super Concentrate Airborne Odor on the shelve above the sink. 2. In the four year old class bathroom there was Clorox Disinfectant Wipes, and Lysol Disinfect Spray on a shelve located to the right of the toilet. 3. The warning labels stated one of the following warnings "keep out of the reach of children', hazardous to humans and animals, and hazardous substance. 4. Staff verified that the hazardous substances were not in a locked location. Plan of Correction: Staff immediately removed the air freshener from the bathroom located near the Circus room, and will move all hazardous substances from the four year old bathroom to a locked location.
Standard #: 22VAC40-185-330-B Description: Based on observation, the center failed to ensure that all fall zones surrounding playground structure were free from barriers or obstacles. Evidence: 1. On the playground there was a white and pink playhouse that measured 5 feet away from the end of the straight blue slide on the playground structure. 2. There was a brown kitchen set that measured 3 feet 10 inches from the bottom of the green slide on the little tyke playground structure. 3. Staff verified that the play kitchen and playhouse were in the fall zones of the slides. Plan of Correction: Staff immediately moved the playhouse and play kitchen out of the fall zones of the slides.
