Comments:
An unannounced monitoring inspection was conducted today between 3:25pm and 5:30pm. There were 20 children in care with two staff including the site director. Children and staff emergency contact information were reviewed. Staff records are stored at a central location and will be reviewed at a later date. Children were observed in play activities, home work, transitions, restroom routines and hand washing. Violations regarding children's records, fire inspection, health inspection and hand washing were identified and appear on the violation notice.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview the center failed to obtain documentation that each child has received the required immunizations. Evidence: 1 - The record for child 1 does not contain any immunization documentation. a. Child 1 has a documented first date of attendance as 9/3/14 2 - Staff 1 confirmed that there is not any immunization documentation in the record of child 1.

Plan of Correction: The documentation will be obtained from the parent as soon as possible.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview the center failed to ensure that each child has a physical examination on file. Evidence: 1 - The record for child 1 does not contain any documentation that a physical examination was completed. a. Child 1 has a documented first date of attendance as 9/3/14 2 - Staff 1 confirmed that there is not any documentation that a physical examination was completed in the record of child 1.

Plan of Correction: The documentation will be obtained from the parent as soon as possible.

Standard #: 22VAC40-185-260-A
Description: Based on documentation review and interview the center failed to provide an annual fire inspection. Evidence: 1 - The most recent fire inspection available for review is dated 9/18/17 and is therefore past due for update. 2 - Staff 1 confirmed that there was not a more recent fire inspection available.

Plan of Correction: Center management will contact the local Fire Marshal and request an inspection.

Standard #: 22VAC40-185-260-B
Description: Based on documentation review and interview the center failed to provide an annual health inspection. Evidence: 1 - The most recent health inspection available for review is dated 3/22/18 and is therefore past due for update. 2 - Staff 1 confirmed that there was not a more recent health inspection available.

Plan of Correction: Center management will contact the local Health Department and request an inspection.

Standard #: 22VAC40-185-500-A
Description: Based on observation and interview the center failed to ensure that children wash their hands after snacks. Evidence: 1 - Child 2 and 3 finished eating snack and immediately began playing with with materials in the room without first washing their hands. 2 - Staff 1 confirmed that the children did not wash their hands after they finished eating.

Plan of Correction: Corrected during the inspection. Going forward center management will ensure that all children wash their hands after eating snacks.
