Comments:
An unannounced renewal study was conducted from 12:30 pm - 3:00 pm. There were 39 children in care being supervised by 6 staff members. The children were observed preparing for nap and napping. Lunch served today is chicken patty on wheat roll, peas, applesauce, and milk. Seven staff records and one agent record was reviewed and in compliance. Ten children's records were reviewed and in compliance. One medication was reviewed. There are currently three staff members on site who are MAT (Medication Administration Training) certified. Standard #: 22VAC40-185-260-B
Description: Based on review of the most recent health department it was determined that the center failed to obtain health inspection annually. Evidence: The most recent health inspection was dated 08/15/2017.

Plan of Correction: Administration will contact the health department to schedule an updated health inspection.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on interview with administration it was determined that not all agents obtained documentation of a criminal history check, central registry check, and sworn statement. Evidence: An Agent listed on the renewal application did not have documentation of a criminal history check, sworn statement, or central registry check on file.

Plan of Correction: Administration will request that the agent obtain a fingerprint check, central registry check, and sworn statement.
