As discussed, please clarify the one physician authorization for one medication with regards to dosage.
An unannounced Renewal inspection was conducted this date. New supervisor on site. A total of 28 children with 2 staff present during the inspection. Children were observed arriving at the program, placing belongings in individual bins and sitting down. Attendance was taken and a review of the daily program was discussed. Children were observed lining up in an orderly fashion for hand washing and bathroom break. Adequate supervision was observed. Snack today was chips and juice pouches. Positive interaction observed between staff and children. Staff reviewed the topic discussed yesterday which was Spain, children took turns remembering customs, food, traditions and dress for Spain. Riddles were told between children in front of the group. A nice transition from snack time to hand wash to homework was observed. A total of 5 children's records were reviewed. MAT (medication administration training) training, Daily Health Observation training and first aid/CPR certifications were checked today. Standard #: 22VAC40-185-240-D-5 Description: Based on review of staff training, at least one staff member with documented daily health observation was not present. Evidence: No documentation found for Staff #1 and #2 that daily health observation training was completed. Plan of Correction: Staff #1 completed the training but the certificate was not at the center. Will obtain copy of certificate and maintain at the center.
