Comments:
An unannounced monitoring inspection was conducted this date. A total of 19 children with 2 staff present today. Children were observed answering to attendance, discussing dinosaurs and transitioning to outdoor play. Observation of bathroom procedures and handwashing and snack time was made. Some repeated violations from the last inspection. MAT (medication administration training) training, Daily Health Observation training and first aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. This inspection began at 2:45pm and ended at 4:45pm. Please email me with questions at: lisa.hudson@dss.virginia.gov

Standard #: 22VAC40-185-130-A
Description: Based on review of child records, documentation that a child was adequately immunized was not found. Evidence: Child #4's record does not contain a copy of current immunizations or a certificate of religious exemption.

Plan of Correction: We will obtain a copy either from the school or the parent.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records some records were found incomplete. Evidence: Child #1's second emergency contact is missing a phone number and address. Child #2's second emergency contact is missing a phone number. Child #4's record did not contain documentation of a health exam.

Plan of Correction: We will obtain this information from the parents and add to the record.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff documentation, emergency contacts for two staff were not found. Evidence: Both staff on site do not have emergency contacts maintained. This is repeated from the last licensing inspection.

Plan of Correction: We will ensure that we document emergency contacts for all staff on duty.

Standard #: 22VAC40-185-90--A
Description: Based on review of 10 children's records, 5 out of 10 records did not contain a signed parent agreement. Evidence: Child #1, 2, 3, 5's records did not contain the program's acknowledgement form including the program's policy on communicable diseases and illness policy.

Plan of Correction: We will have the parents sign this acknowledgement form and retain a copy in the records.

Standard #: 22VAC40-185-240-D-5
Description: Based on review of staff documentation, no documentation was found that at least one staff has daily health observation training. Evidence: Both staff on cite did not have any certification of completion of daily health observation training.

Plan of Correction: One staff has completed this training and will obtain and maintain the certificate on site.

Standard #: 22VAC40-185-550-M
Description: Based on staff statements, a written record of serious and minor injuries is not maintained. Evidence: Staff #1 stated that all incident reports are in the main office and not maintained at the center.

Plan of Correction: We will obtain all injury reports to date and maintain them at the center.

Standard #: 22VAC40-185-560-F
Description: Based on observation of the physical plant, the menu posted was not current. Evidence: The snack menu posted today was for the month of August.

Plan of Correction: We will post the current snack menu.
