Technical Assistance:
This inspector spoke with the Director about 22VAC40-185-240 Staff training and development during this inspection.

Comments:
An unannounced monitoring inspection and facility tour was conducted from 10:15am-11:50am. At the time of inspectors' arrival there were 7 infants in care with 2 staff members. Children were observed participating in infant activities, interacting with staff, feeding, diapering, transitioning between activities, and nap. A sample of 5 children's records and 4 staff records were reviewed. An injury report was also reviewed. The Staff reported that there are no medications being administered at this time. Areas of noncompliance are detailed in the violation notice and were discussed with the Director at the conclusion of this inspection.

Description: Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #4 did not contain documentation of immunizations. 2. The Director confirmed this documentation was not available during this inspection.

Description: Based on record review and interview it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #2(enrolled 1/15/18) did not contain current immunization documentation. The documentation available was dated 1/15/18. 2. The Director confirmed this documentation was not available during this inspection.

Description: Based on record review and interview it was determined the center did not ensure each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: 1. The record for staff #4(hired 11/30/17) did not contain documentation of a tuberculosis screening. 2. The director confirmed this documentation was not available during this inspection.

Description: Based on record review and interview it was determined the center did not ensure that there was annual approval from the health department. Evidence: 1.
