An unannounced inspection was completed on 09/25/2018 from 9:20 A.M.-11:45 A.M. At the time of the inspection 41 children were in care with six staff and one administrator present. Five children records, five staff records, Injury Prevention Plan, emergency plan, first aid kits, emergency supplies, and posted required information were reviewed. Children were observed during free choice activities which included painting, blocks, dramatic play, music and movement, story time, handwashing, snack time, and behavioral guidance. There was a wide variety of educational activities available and positive interactions observed between the teachers and the children. The exceptions to compliance are noted on the violation notice. Standard #: 22VAC40-185-90--A Description: Based on review of children's records, the center failed to ensure that all components of the written agreement were kept on file. Evidence: 1. Five out of five children records did not contain the parental agreement stating that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center. 2. Five out of five children records did not contain the statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. 3. Staff verified that a written parental agreement was not on file for all five children. Plan of Correction: The director immediately typed up the missing agreements to send out to current students tomorrow, and will revise the current contract for future enrollments.
