22VAC40-191 Background Checks (22VAC40-191)

The licensing inspector conducted an unannounced renewal inspection on 09/20/2018 from 10:15am to 5:16pm. The children were observed playing games, exercising, playing on the playground, eating lunch, and napping. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. Eleven injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 08/08/2018 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 08/08/2017 Last health inspection: 10/20/2017 Today, the following child to staff ratios were observed: Rm. 1 (Twos) ? 11:3 Rm. 2 (Twos and Threes) ? 13:3 Rm. 4 (Twos and Preschool) ? 12:2 Rm. 5 (Twos and Preschool) ? 13:2 Rm. 6 (Twos and Preschool) ? 13:2 Rm. 7 (Preschool) ? 12:2 Rm. 8 (Preschool) - 6:1 Rm. 9 (Preschool) - 10:1 Rm. 10 (Preschool) - 12:2 Rm. 11 (Preschool) - 8:2 Rm. 12 (Preschool) - 8:1 Rm. 13 (Preschool) - 15:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. The Kids Place art room contained three buckets of an unlocked hazardous substance located on the top of a 7ft. shelf. The substance's containers listed the following warnings: Keep out of reach of children, causes skin irritation, causes skin to burn. 2. Room #12 had an unlocked closet with cleaners located on a shelf out of the reach of children.

Plan of Correction: Corrected during the inspection and the EC Director will review with all staff.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center did not ensure that a written record with the required information was maintained for children's serious and minor injuries. Evidence: Seven out of the eleven injury reports reviewed by the inspector did not contain documentation of the date and time the parent was notified.

Plan of Correction: EC Director will share new form with all required information. Form will be ready on 9/21/18.

Standard #: 22VAC40-185-560-K
Description: Based on observation and interviews, the center did not ensure that tables were sanitized before and after each use for feeding. Evidence: The inspector observed the children eating snack, then playing with games and toys. The inspector did not observe the table being sanitized after snack. Two staff members informed the inspector that the tables were not sanitized before and after the children ate snack.

Plan of Correction: EC Director will review with Kids Place Director on 9/21/18.
