Comments:
An unannounced monitoring inspection was initiated on September 24, 2018 and completed on September 26, 2018. Six staff were observed with sixty-five children at the onset of inspection. Six staff records and six children's records were reviewed.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review, provider did not ensure 1 of 6 staff has a TB test or screening within 21 days of beginning employment. Evidence: On September 24, 2018, the record for staff #5 (start date May 7, 2018) did not have a TB test or screening.

Plan of Correction: Immediately sending staff to get the TB test before they begin to work.

Standard #: 22VAC40-185-280-B
Description: Based on observation, facility did not ensure hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: On September 24, 2018, two bottles of disinfectant spray labeled "Caution....keep out of reach of children" were observed on a table and an open shelf in the program director's office. The office was not locked.

Plan of Correction: Chemicals will be kept locked.

Standard #: 22VAC40-185-510-J
Description: Based on observation, facility did not ensure medication, except for those prescriptions designated otherwise by written physician's orders, are kept in a locked place using a safe locking method that prevents access by children. Evidence: On September 24, 2018, an albuterol inhaler was observed sitting on a desk in the program director's office.

Plan of Correction: All medication will be kept locked and advise staff not to accept medication without proper documentation.

Standard #: 63.2-1720.1-B-2
Description: Based on review, facility did not ensure 5 of 6 staff records has a Office of Background Investigations (OBI) letter stating eligibility for hire in response to the national fingerprint check prior to beginning employment. Evidence: On September 24, 2018, the following was found during record review: 1. The record for staff #1 (start date May 8, 2018) has a OBI letter of eligibility for hire dated May 14, 2018. 2. The record for staff #2 (start date May 22, 2018) has a OBI letter of eligibility for hire dated May 29, 2018. 3. The record for staff #3 (start date May 31, 2018) has a OBI letter of eligibility for hire dated June 1, 2018. 4. The record for staff #4 (start date May 22, 2018) has a OBI letter of eligibility for hire dated May 25, 2018. 5. The record for staff #5 (start date May 7, 2018) has a OBI letter of eligibility for hire dated May 21, 2018.

Plan of Correction: Will make sure we complete the background checks before starting training for our staff.
