A Subsidy Health and Safety inspection was conducted on September 24, 2018 and September 26, 2018 to review supplemental health and safety requirements. Six staff were present with sixty-five children at the onset of children. Six children's records and six staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-B Description: Based on review, 2 of 6 staff records did not have documentation of completed Virginia Preservice Training for Child Care Staff within 90 days of beginning employment. Evidence: On September 24, 2018, the records for staff #3 (start date May 31, 2018) and staff #6 (start date May 24, 2018) did not have documentation of completing Virginia Preservice Training. Plan of Correction: Will get a copy of the training certificate from the staff to place in the file. If staff has not completed the training, it will be done immediately.
Standard #: VENDSUB-000-012-E-2 Description: Based on review, facility did not ensure 1 of 6 staff are currently certified in first aid. Evidence: On September 24, 2018, the record for staff #2 does not have documentation of first aid certification. Plan of Correction: She will complete the course.
