Comments:
An unannounced renewal inspection was conducted today from 7:45am-9:10am. There were 54 school age children directly supervised by 4 staff. The physical plant, 4 staff records, 5 children?s records, 3 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing in the gym, and socializing. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-40-J
Description: Based on record review, the injury prevention procedures were not updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: There was not an updated injury prevention plan on file.

Plan of Correction: We will conduct the review annually going forward.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review, the staff did not request at least annually parent confirmation that the required information in the child's record is up to date. Evidence: Two of five records reviewed did not have confirmation that information in the child's record was up to date. Child 1 and Child 2 files were last updated in 9/2016.

Plan of Correction: Parents were asked for the update but we neglected to get signatures. I will have parents sign files today.
